  2. Halachos of Brachos Handbook p. 51 explains that if you want the cake then the ice cream is enhancing the cake and is considered primary. However, if you primarily want the ice cream then a piece of cake should be broken off to make mezonot and then a shehakol on the ice cream. Laws of Brachos (Rabbi Forst, p. 370) agrees.
  3. Halachos of Brachos Handbook p. 51 writes to recite two brachot over an ice cream cone, one for the cone and one for the ice cream since the cone enhances the ice cream but is distinct.
